In order to serve you better we have added a basic list of some of our service fees and the option to pay your account balance or pre-pay for your services.  On-line payments can be made at the bottom of this page.


Office Visit:

Basic Office visit with 1 or 2 chief concerns ~ $105

Detailed Office visit with 3 or 4 concerns ~ $150

Comprehensive Office visit 4 or 5 concerns or complex disease/disorder ~ $250

Fibromyalgia / Chronic Fatigue Office Visit ~ $500 ( additional fee for saliva and blood testing)


Sick Visit

UTI ( Urinary Track Infection) ~  $60

PAP Smear ( includes screening for HPV) ~  $140 

Diabetes Management (Does not include lab work) ~ $130  

Hypertension Management ( Includes EKG, Chest X-ray and Urine analysis) ~  $230  first visit

Breathing Treatment~$150

A comprehensive regular physical examinations includes: chest x-ray, urine analysis, EKG, vision test, review of systems and a general health panel screening.  Additional lab work can be added based on your individual needs.  Our in house lab is Spectrum Labs and they accept most insurance plans.  Our self pay patients will receive up to a 40% discount on their lab work when it is paid on the date of service.

Physical Examination ages 21- 39 ~ $330 

Physical Examination ages 40 - 64 ~ $350

Physical Examination ages 65 and Over ~ $370 



Holistic Evaluation ~ $1100
This evaluation is paid completely out of pocket and well worth it!  Your screening will include: Full body thermography, comprehensive bio-meridian testing, nutritional deficiency testing, food allergy testing, chiropractic evaluation/treatment, body composition analysis, five year diabetic risk screening and a general health panel.   

 

Medical Examination for Commerical Driver Fitness Determination  ~ $115
This is the examination required by the DOT (Department of Transportation) includes certificate



Immigration Examinations ~ $250   If you are up to date with all of your vaccinations


Weight Loss Program ~ $1180  Monthly payments of $295 available.
