Immigration Examination
 

Thank you for selecting The Robbins Health Alliance for your Immigration needs. Dr. Lisa Robbins has been a civil surgeon for several years authorized to conduct your examination for your dream of becoming an America citizen!  Your  immigration physical will cost only $250.00 if all of your vaccination records are up to date.  Because we do not want you to repeat any vaccinations you may have already received, we urge you to bring any vaccinations records you may have. Same day appointments are available except on Thursday.
 

 Insurance DOES NOT cover the examination however check with your primary doctor for the required vaccinations.  Additional lab work can be conducted upon request.  Your examination is valid for one year from the date you receive it.   

 
 
 

          You will get the following for $250 FEE:

          Immigration Examination

          Chest X-Ray (if needed)

          Syphilis, Measles, Mumps, Rubella, and Varicella Screening

          TB (Tuberculosis) skin test *                                                                     

          Form I-693 and its supplemental form results in a sealed envelope

 

 

 *The Tuberculosis test requires you to return to our office with 2 to 3 day to read the results.  At that point a chest X-ray may be neccessary.

 

 

 

    

Vaccinations we offer:
Varicella (chicken pox) vaccination - $150.00

TD (Tetanus and Diphtheria) vaccination - $50.00

Tdap (Combined Tetanus, Diphtheria and Acellular Pertussis) vaccination - $75

Pneumoccal vaccination (50 years and older) -  $50.00. 

If you need an Influenza vaccination (only during flu season September to February)  and other vaccinations we recommend obtaining them a clinic inside of Walgreens, CVS or the Health Department. 

 


Vaccination Required by USCIS:

  • Rotavirus vaccine should be given orally to children 6 through 32 weeks of age.
  • Hepatitis A vaccine should be given to children 12 through 23 months of age.
  • Meningococcal  vaccine should be given to persons 11 through 18 years of age. 
  • Human papillomavirus vaccine should be given to females 11 through 26 years of age.
  • Zoster vaccine should be given to persons 60 years of age or older.
  • Hepatitis B vaccine should be given from birth through 18 years of age.
  • Influenza vaccine should be given annually to children 6 through 59 months of age.  It continues to be required for adults 50 years of age or older.
  • The adolescent and adult formulation of tetanus and diphtheria toxoid and acellular pertussis (Tdap) vaccine is given as a single (booster) dose for persons 10 through 64

What to Bring with you:

  • Any documentations from Homeland Security concerning your examination
  • Vaccination records (if available)
  • Passport or government issued photo identification
  • Alien Number (if issued)
  • Payment

 

It will take 2 – 4 days for your laboratory results to be returned to our office.  We will call you when your laboratory results are in and your paper work is completed.  We ask that you provide us with (2) two possible contact numbers.  Once you receive the completed paper work in a sealed enveloped you cannot open it.  It must be given to the Homeland Security representative in our sealed and signed/stamped envelope.

 

Any additional paperwork after your initial examination will cost $50.
